These terms and conditions apply to all treatment agreements between Shane Gnade Physiotherapy and the patient. By making an appointment, you agree to these terms.

2. Treatment agreement
The treatment agreement comes into effect the moment a patient makes an appointment with Shane Gnade Physiotherapy. This agreement is governed by the Dutch Medical Treatment Agreement Act (WGBO).
The physiotherapist commits to providing responsible care in line with professional standards. The patient commits to providing all relevant information necessary for proper treatment.
3. Appointments and cancellation
We ask you to cancel appointments at least 24 hours in advance. In the case of late cancellation or a missed appointment, we are required to charge for the reserved time:
- Cancellation less than 24 hours in advance: 50% of the treatment fee
- No-show without cancellation: 100% of the treatment fee
No-show fees are not reimbursed by health insurers and are payable by the patient.
4. Fees and payment
The fees charged are in line with the NZa rates (Dutch Healthcare Authority). Current rates are available on request from the practice.
Shane Gnade Physiotherapy bills your health insurer directly where a contract is in place. Any excess (eigen risico) and non-reimbursed treatments are charged directly to the patient.
Invoices must be paid within 14 days of the invoice date. In the event of late payment, Shane Gnade Physiotherapy reserves the right to suspend further treatment.
5. Health insurance reimbursement
Shane Gnade Physiotherapy has contracts with all Dutch health insurers. Reimbursement for physiotherapy depends on your supplementary insurance. We recommend checking with your insurer in advance regarding the level of reimbursement.
The excess (basic insurance) does not apply to physiotherapy, unless the condition is on the chronic conditions list.
6. Patient rights
As a patient, you have the following rights:
- Right to information, you are entitled to clear information about your condition and treatment
- Right to consent, treatment only takes place with your consent
- Right of access, you have the right to access your file
- Right to privacy, your data is handled confidentially in accordance with the GDPR
- Right to a second opinion, you may always request a second opinion
7. Liability
Shane Gnade Physiotherapy is liable for damage that is the direct result of an attributable shortcoming. Liability is limited to the amount paid out by the professional liability insurance.
Shane Gnade Physiotherapy is not liable for damage to or theft of patient belongings at the practice.
8. Complaints
Do you have a complaint about treatment or conduct? We ask that you first raise this with the physiotherapist directly. If this does not resolve the matter, you can file a complaint with:
- SKGE (Foundation for Complaints and Disputes in Primary Care) via www.skge.nl
- Royal Dutch Society for Physical Therapy (KNGF) via www.kngf.nl
9. Records and privacy
Shane Gnade Physiotherapy maintains a patient file in accordance with the WGBO. This file is retained for at least 15 years. For more information on how we handle your personal data, please see our privacy policy.
10. Applicable law
Dutch law applies to all agreements between Shane Gnade Physiotherapy and the patient. Disputes are submitted to the competent court in the district where the practice is located.
11. Changes
Shane Gnade Physiotherapy reserves the right to amend these terms and conditions. The most current version is always available on our website. Changes do not apply retroactively to ongoing treatment agreements.
